Physician's Office Visits

$20.00 Co-pay inside the 4- Most  and First Health PPO Networks regardless of physician's specialty.  No HMO, no referrals, no primary care physicians.  Plans also available needing no PPO Network and payable to any doctor and any hospital.

Prescription Drug Card

$15.00 Copay for generic medication.  Name brand $25.00 co pay.   Prescription plans vary per different companies.  Prescriptions covered from day one regardless of pre-existing health condition.

Accident Coverage

$0 Deductible, 100% coverage up to $500.  After $500 each expense goes toward the deductible. 

Preventive Coverage

Pay your office copay then 100% coverage to $100 per insured per year.  Children's well visits are paid with no yearly limitation to age 6.
